Dental Arts Practice brings a prosthodontist, a board-certified endodontist, and a board-certified periodontist together in one office. We welcome referrals from general dentists across the San Gabriel and Pomona valleys.
What you can refer
- Prosthodontics: complex restorative cases, full-mouth rehabilitation, dentures, and implant restorations
- Implant surgery, bone grafting, and full-arch (All-on-4 style) cases
- Endodontics: root canal treatment, retreatment, and diagnosis of difficult pain
- Periodontics: gum disease treatment, gum grafting, and implant-site development
- CBCT (3D) scans
How to refer
- Download and complete the specialist referral form.
- Send it with any radiographs through an approved secure portal, encrypted email, or HIPAA-compliant fax.
- For urgent cases, call us at (909) 598-9097 so we can arrange timely care.

Working together
We keep you informed about your patient's evaluation and treatment, and we focus on the care you referred them for. Patients are welcome in English, Spanish, Chinese, Farsi, Filipino, Arabic, and Turkish, and we accept Medi-Cal Dental and all PPO dental plans.
