For referring dentists
If you are a dentist referring a patient for prosthodontics, endodontics, periodontics, implant surgery, or a CBCT scan, please use our specialist referral form. More about working with us is on our referring dentists page.

For patients
New and existing patients do not need to complete forms in advance. At your first visit, our front desk team will walk you through any intake paperwork, insurance forms, and consent forms in person.
